		Why a Transition Information Pack? The Transition Information Pack is designed for all post 16 students with a Statement or School Action Plus leaving school, going to a training provider or college. It is designed to be easy to fill in, referencing other documents. It is important that the parent/carer and young person (if appropriate) sign the last page to agree permission for information to be passed on.
